Aluminum struck medal with obverse showing bust-length portrait of man wearing uniform with legend: "JOHN GRAVES SIMCOE . LIEUT . GOVERNOR ./A.D. 1791 A.D. 1796." Reverse bears inscription at center: "REPRESENTATIVE/SYSTEM PROCLAIMED/KINGSTON, JULY 16 1792/FIRST PARLIAMENT OPENED/NIAGARA, SEPTEMBER 17 1792/CENTENNIAL CELEBRATED/1892'; surrounding legend: "UPPER CANADA/SINCE 1867 ONTARIO."
