A blackish gold metal medal with a pin bar embossed "Faithful Service", attached to a dark blue ribbon linked to a dangling medallion by a chain link. The medallion is embossed with an eight pointed star around a raised frame embossed "Metropolitan Life Insurance Co." encircling a letter "M".
