Bronze struck medal with obverse showing neo-Gothic church building with legend: "CATHEDRAL CHURCH OF SAINT JOHN THE DIVINE/IN THE CITY AND/DIOCESE OF NEW YORK." Reverse shows almond-shaped seal of New York Diocese flanked by two crosses; legend: "CONSECRATION OF THE CATHEDRAL CHOIR, S.SAVIOR'S CHAPEL AND S.COLUMBA CHAPEL/APRIL XIX, MCMXI."
