Gilt aluminum struck medal with obverse depicting windmill surrounded by legend: "PORTSMOUTH, RHODE ISLAND/1638 - 1938". Reverse bears round medallion at bottom, inscribed: "TOWN OF PORTSMOUTH, NEWPORT CO. R.I./INCORPORATED/1640," laurel sprays on either side, dates on scroll above: "1638-1938"; inscription above: "TERCENTENARY/OF/PORTSMOUTH, R.I./JOHN CLARKE/FOUNDER."
