This tiny wedding or anniversary cake box is cream colored with silver monogramed initials on the top lid that read "P.L." inside a diamond design. It opens with a hinge like a book and ties closed with attached ribbons. Inside are doilies and the words "Madame Blanche/ 249 East 57th Street/ New York."

On the sides, the word "Balsam" and a partial word "[Pul]monary". Below are the words "Vegetable" and "Carter's Balsam", all in reverse.
